What is going on

Condemned pole 119 on Buninyong feeder BUN-12, work order WO-26-4410, is to be replaced on Tuesday 16 September with a 22 kV and low voltage outage from 8:00 am to 2:00 pm. Access request EAA-26-2210 is in the register and switching program SP-26-0921 is being checked. The pole is on the Midland Highway shoulder, requiring traffic management, and the low voltage mains supply a dairy that has been told to run its generator. The crew of four includes a second year apprentice, and the elevated work platform and a pole borer are on site. A second 22 kV feeder crosses the road two spans away and remains live.

I want you to act as access recipient for the pole 119 replacement on Buninyong feeder BUN-12, work order WO-26-4410, next Tuesday. The access request EAA-26-2210 is in the register and the switching program SP-26-0921 is with the controller for checking. Prepare the tailgate briefing and job risk assessment, including the traffic management on the Midland Highway and the elevated work platform rescue plan, take the authority from the controller with a proper read back, brief the crew, and cancel it correctly when we are done. Read the access authority procedure first.

Your brief

Prepare the crew and site for the outage, take the electrical access authority from the network controller with a correct read back, hold it responsibly for the duration of the work including the working earths and the live adjacent feeder, and cancel it correctly so that restoration can proceed, all in line with the access authority procedure.

Read the access authority and permit to work procedure and the electrical access authority form, and review EAA-26-2210, SP-26-0921 and WO-26-4410 in the registers.
Prepare the tailgate briefing and job risk assessment, including traffic management, the elevated work platform rescue plan, the pole borer near the live crossing feeder, the apprentice's supervision and the dairy's generator.
Check the authorised persons register for every crew member's authorisation and rescue currency and record the categories on the risk assessment.
Take the access authority from the network controller by read back, complete the access authority form with the apparatus, isolation points, earths and adjacent live apparatus, and record the issue time.
Brief the crew on the limits of the access, sign every worker on, and apply working earths at the work site before any conductor is touched.
Describe the sequence for the pole replacement and conductor transfer under the authority, including the point at which the pole is supported before conductors are cut.
At completion, confirm all workers clear, remove working earths, sign workers off, and cancel the authority with the controller by read back, recording the cancellation time on the form and the register.
Update work order WO-26-4410 with hours, the authority number and any hazard or near miss for the incident register.
