Audit the permit system after two near misses

You are the HSE Advisor reporting to the HSE and Quality Manager.

What is going on

Two near misses this month involved the permit system: the lube oil pump missing from the BFP-2B isolation list, caught at walk down, and a contractor scaffolder entering the GT1 fuel gas skid area without checking the permit board while the skid was depressurised for instrument work. The scaffolder event has no corrective actions recorded. Jordan Meyer wants the September permit audit done early, with every permit issued this month reviewed against the policy and the multiple energy source procedure, breaches recorded as incidents, and corrective actions with owners before the October outage brings 90 contractors to site.

Jordan Meyer, HSE and Quality Manager
Subject: Permit walk down near miss and the September permit audit

Two near misses this month were about permits: the missing lube oil isolation on BFP-2B and the scaffolder walking into the fuel gas skid area. I want the September permit audit done early. Please review every permit issued this month against the Permit to Work and Isolation Policy and the multiple energy source procedure, record findings as incidents where a rule was breached, and give me the corrective actions with owners. Include the scaffolder event, which still has no actions recorded.

Your brief

Audit every September permit in the register against the policy and the procedure, record each breach or gap in the incident register with corrective actions and owners, complete the actions for the scaffolder near miss and prepare the audit report and a contractor briefing for the October outage.

Work through it

You do not need to do it all. Doing a few of these will show you what the work is like. Your ticks and notes stay on this device.
