What is going on

At 10:40 am on 2 September a member of the public called the control centre to report a loud hissing and a strong smell of gas outside a house on Wilson Street, Wodonga. A fencing contractor had augered a post hole and struck the 63 millimetre polyethylene main. The control centre dispatched you as Priority 1. On arrival you found gas venting freely from a broken pipe, the contractor's crew still on site, and a resident standing beside the excavation. Twelve customers on the street depend on that main. The escape log and the incident register hold only the dispatch entry; the rest of the response now needs to be carried out and recorded properly.

Kirra Thompson, Emergency Response Coordinator
Subject: Wodonga third party strike: complete the response record

The fencing contractor strike on Wilson Street is still open in the Gas Escape and Leak Survey Log and the incident register. I need the response written up properly before the regulator asks for it. Please work through the first response procedure, complete the Incident and Hazard Report Form from Aisha's notes, and update both records with the readings, times and make safe actions. Flag anything that did not follow the procedure so we can address it in the debrief on Thursday.

Your brief

Work through the first response procedure for this event: assess and set site control, make safe, isolate under permit, arrange repair and restoration, and complete every record. Your records must be good enough to support the regulator notification and the debrief.

Work through it

You do not need to do it all. Doing a few of these will show you what the work is like. Your ticks and notes stay on this device.

What someone in this role would produce

Printing this is a record of what you explored. It is not a certificate or a qualification.
