Plan and authorise an isolation affecting more than 50 customers

What is going on

Liam Fitzgerald has applied for permit PTW-26-0424 to replace a leaking valve on the 150 millimetre steel main on the Princes Highway service road at Morwell. The isolation will cut supply to 68 customers including a bakery, a childcare centre and a small hospital kitchen. The Permit to Work and Isolation Policy requires manager approval for any outage over 50 customers, a proven isolation plan, control centre notification and a relight plan for every customer. The application is missing the customer count justification, the second isolation point is not identified, and no traffic management plan is attached. Sam Whitlock wants the permit ready for issue on Monday 21 September.

• Read the Permit to Work and Isolation Policy and the Procedure: Isolations Under the Permit to Work System and list every requirement that applies to an outage of more than 50 customers.
• Review PTW-26-0424 in the Permit to Work Register and record each gap you find against the policy and procedure.
• Complete a corrected Permit to Work Application naming both isolation points, the proving method and readings expected, the purge plan and the fire precautions.
• Prepare a customer notification list and relight plan for the 68 customers, prioritising the hospital kitchen, the childcare centre and the bakery, and state how relights will be recorded.
• Draft the approval request to Tom Halloran with the outage duration, the risk controls and the contingency if the valve cannot be replaced in the window.
• Update the permit in the register to Issued with the approval recorded, and add a work order in Field Work Orders for the valve replacement linked to the permit.
• Prepare a Job Safety Analysis for the excavation and valve replacement, including excavation support for a trench beside a highway service road.
• Write a short note to the control centre stating the isolation and restoration windows and what the control centre must do at each step.
