What is going on

At 9:15 pm on 8 September the SCADA system raised a low outlet pressure alarm on district regulator DR-302 at Pakenham. Outlet pressure was falling towards the low limit that would affect supply to around 3,000 customers on a cold night. The operator on duty acknowledged the alarm, dispatched Ravi Prasad, and received a call back at 10:05 pm that the pilot line had iced and had been cleared. The next morning Darren Okafor found that the log entry was incomplete, the asset register note did not mention the follow-up, and the handover to the night shift did not record the event. He wants you to reconstruct the record correctly and to explain to the shift leaders what a complete entry looks like.

Darren Okafor, Control Centre Manager
Subject: Afternoon shift handover and DR-302 alarm

The low outlet pressure alarm on DR-302 at Pakenham last night was cleared as an iced pilot line but I want the log entry and handover reviewed. Please check that the alarm, dispatch, field feedback and restoration were all recorded in the correct order, that the asset register note matches the field report, and that the shift handover captured the follow-up. If you find gaps, correct the records and write a short note for the shift leaders.

Your brief

Review the alarm event from acknowledgement to restoration, correct and complete every record in the escape log and the asset register, and write a handover note that shows the shift leaders the standard expected for every alarm.

Work through it

You do not need to do it all. Doing a few of these will show you what the work is like. Your ticks and notes stay on this device.
