Audit the training and authorisation register before the safety case review

What is going on

The quarterly safety case review on 30 September will examine whether the business is meeting its commitment that only authorised people perform controlled tasks. Helen Marchetti has asked for an audit of the Training and Authorisation Register against the permits issued in September and the emergency response roster. Six apprentices and two cadets started on 14 September, the polyethylene fusion reassessments are not due until October, and the August permit audit found record keeping problems at the Traralgon depot. You have the register, the permit register, the roster and the incident register. Sam Whitlock wants a one page summary with corrective actions that Helen can table at the review.

Your brief

Audit the register against the permits and the roster, identify every expired, expiring or missing authorisation and every permit signed by someone without a current authorisation, and prepare the summary and corrective actions for the safety case review.

• Read the Training and Authorisation Register document and the Permit to Work and Isolation Policy and write the audit criteria you will apply.
• Review every permit in the Permit to Work Register for September and check the holder against the authorisation categories and expiry rules in the register document.
• Review the emergency response roster against the first response authorisation and record any responder whose authorisation expires before the end of the year.
• Record any breach or gap you find as a Hazard or Near miss in the HSE Incident and Hazard Register with a corrective action, owner and date.
• Prepare the one page audit summary for the General Manager covering method, findings, corrective actions and the assurance you can give the review.
• Draft a note to depot supervisors explaining how to check the register before assigning a controlled task to a new apprentice or cadet.
• Prepare the audit finding for the management system review using the Gas Industry Technical Report Template so it can be filed in the controlled library.
