Investigate the ramp 4 vehicle interaction high potential incident

What is going on

On 7 September 2026 a light vehicle entered the ramp 4 haul road at the Deepridge open cut without positive communication and came within 20 metres of a loaded rigid haul truck on the descent. The truck operator stopped and there was no contact. It is a high potential incident under the vehicle interaction principal hazard management plan and the regulator has been notified. The scene data has been downloaded. The light vehicle driver was a contractor surveyor on his second day on site. The HSE and Training Manager wants the investigation done using the incident cause analysis method and the Incident Investigation Report Template, with the draft report and actions within seven days.

Your brief

Lead the investigation of the ramp 4 incident: gather the facts and records, interview both drivers, build the timeline, identify the failed controls and their organisational causes, write the report on the template and enter the corrective actions in the Hazard and Incident Register.

• Read the Incident Investigation Report Template and the Health and Safety Policy and list the records you must collect, including the prestart record, Take 5, radio log, plant data and induction records.
• Check the contractor surveyor's induction and light vehicle authorisation in the Training and Authorisation Matrix and note what you find.
• Prepare interview plans for the truck operator and the surveyor, to be interviewed separately, with open questions that establish facts.
• Build the timeline of the event from the plant data, the radio log and the interviews.
• Identify the absent or failed controls against the Mobile Plant Prestart and Operation Procedure and trace each to an organisational cause.
• Rate the actual and potential consequence on the risk matrix and record it on the template.
• Write corrective actions in order of the hierarchy of controls, with owners and due dates, and enter them in the Hazard and Incident Register.
• Draft the prestart learning to be shared across all three sites.
