Report and investigate two depot incidents
You are the Occupational Health and Safety Coordinator.
What is going on
Two events at the Kellerton depot need to be handled properly. On Creek Road the grader clipped a marker post while turning on a narrow verge, with no injury but damage to the post and a scrape on the blade. At the Church Street drain pit a crew member leaned into the pit to clear leaves without a gas test, which is a confined space near miss. Both were reported verbally to Hemi Tauroa but the forms have not been completed. Under the Occupational Health and Safety Policy both must be in the Incident and Hazard Register within 24 hours. Hemi wants corrective actions agreed and a toolbox talk ready for Monday's pre-start.
Subject: Two incidents to write up
We had the near miss at the Church Street pit yesterday and I clipped a marker post with the grader on Creek Road last week. Both need to go through properly. Please complete the Incident and Hazard Report Form for each from the crew's notes, enter them in the Incident and Hazard Register, and work out the corrective actions with me. The pit one worries me, nobody should be leaning into a pit without a gas test. Put a toolbox talk together for Monday's pre-start.
Your brief
Complete the Incident and Hazard Report Form for each event, enter both in the Incident and Hazard Register, decide whether either is notifiable, investigate the confined space near miss, agree corrective actions with the Works Supervisor and prepare a toolbox talk on pit and confined space entry.
Work through it
You do not need to do it all. Doing a few of these will show you what the work is like. Your ticks and notes stay on this device.
What someone in this role would produce
- Two completed Incident and Hazard Report Forms
- Two register entries with corrective actions
- Investigation summary for the confined space near miss
- Toolbox talk outline and sign-on sheet
Printing this is a record of what you explored. It is not a certificate or a qualification.
