Conduct an administrative inquiry into an unescorted visitor
You are the Inquiry Officer appointed by the General Manager.
What is going on
On 30 August a visitor was found walking unescorted between the depot office and the vehicle compound at the Rockhampton depot. The event was recorded in the WHS Incident and Hazard Register, the visitor was escorted out and the security officer was told within 24 hours. The customer has now asked for a formal administrative inquiry into how it happened and whether the escort requirement in the Security Policy is being followed at that site. Marcus Thorne has appointed you as inquiry officer with Tom Whitlock as adviser. The visitor register, the key and access card register, the site induction records and the staff involved are available to you.
Subject: Unescorted visitor at Rockhampton, formal inquiry
The unescorted visitor event at the Rockhampton depot on 30 August was closed quickly, but the customer has asked for a formal administrative inquiry into how it happened and whether our escort procedure is being followed. I am appointing you as the inquiry officer. Work with Tom, review the security policy and the visitor and key registers, interview the people involved, establish the facts and give me findings and recommendations that I can send to the customer. Treat everyone fairly and record every step.
Your brief
Conduct a fair, evidence based administrative inquiry that establishes what happened, why the escort requirement failed, whether the policy and procedure are adequate, and what corrective actions are recommended, and report to the General Manager in a form suitable for the customer.
Work through it
You do not need to do it all. Doing a few of these will show you what the work is like. Your ticks and notes stay on this device.
