Review a wellness plan with the person who owns it
You are a Recovery Support Worker in the Mental Health and Wellbeing team.
This mission touches on a mature theme. It is fictional. If it brings up something hard for you, you can stop at any time and talk to someone you trust.
What is going on
Ben Carmody is 38 and has received psychosocial support from Willow for eight months. He has a wellness plan and a crisis plan, a GP mental health treatment plan, and attends the Friday peer group. Last week he had a difficult time, missed the group and phoned Sam Okafor to talk through his early warning signs. He did not express any safety concerns. Ben has also asked to become a peer volunteer. Ruth Abela has asked you to visit Ben this week, review his plans with him, check in with his GP with his consent and start a conversation about the peer volunteer pathway.
Subject: Ben Carmody wellness plan and peer volunteer request
Ben Carmody had a hard week, missed the peer group and phoned Sam to talk through his early warning signs. He has also asked to become a peer volunteer, which is wonderful. I would like you to visit Ben this week, review his wellness and crisis plan with him using the Individual Support Plan Template, check in with his GP with his consent, and start a conversation about what being a peer volunteer would involve. Please read his progress notes and the Person-Centred Practice and Rights Policy first, and remember the plan is his, not ours.
Your brief
Visit Ben, review his wellness and crisis plan in a way that keeps him in the lead, respond to the change in his wellbeing early, coordinate with his GP with consent, and explore the peer volunteer role with him honestly.
Work through it
You do not need to do it all. Doing a few of these will show you what the work is like. Your ticks and notes stay on this device.
What someone in this role would produce
- Visit plan with open questions
- Updated wellness and crisis plan agreed by Ben
- Consent record and GP summary
- Progress note and Client Register update
- Escalation pathway note
Printing this is a record of what you explored. It is not a certificate or a qualification.
