Run an NDIS plan review the right way
You are a Support Coordinator in Disability Support Services.
What is going on
Aisha Rahimi is 27, receives daily living supports morning and evening, uses a hoist with two workers and has epilepsy. Her family speak Dari and her plan nominee has complained that the last plan review went ahead without an interpreter, so the family felt decisions were made without Aisha. Her plan review is due on 28 September. Aisha has told workers she wants to spend more time in the garden and would like to attend a women's swimming session at the local pool. Her NDIS plan includes community access hours that are not being used. Anjali Deshpande wants this review to be a model of how Willow does it.
Subject: Aisha Rahimi plan review with interpreter this time
Aisha's plan review is due on 28 September and her nominee has made a complaint that the last meeting went ahead without a Dari interpreter. This time it must be done properly. Please book an interpreter, prepare the Individual Support Plan Template with Aisha's current goals from her NDIS plan, and read the progress notes from the last month so you can speak to what has been working. The two-worker hoist transfer and the seizure management plan must be reflected in the plan. Record the review in the Client Register and close the loop on the complaint in the Feedback Register.
Your brief
Prepare and run Aisha's plan review with an interpreter, build a support plan around her own goals and her NDIS plan, make sure her safety needs are documented for every worker, and close out the complaint so the family can see what changed.
Work through it
You do not need to do it all. Doing a few of these will show you what the work is like. Your ticks and notes stay on this device.
What someone in this role would produce
- Review agenda and interpreter booking note
- Completed Individual Support Plan agreed by Aisha and her nominee
- Roster request for community access
- Complaint response and updated register entry
- Reflection on what made this review person-centred
