Investigate a dog bite incident and close out corrective actions
You are the WHS Coordinator in the Business Support team.
What is going on
On 8 September an animal attendant was bitten on the hand while removing a food bowl from a seized mastiff cross held in the observation block. The bite broke the skin and required medical review. The dog had been seized following a dog attack complaint, but the bite risk flag was not on the kennel card at the time. The Operations Manager has assigned you the investigation and expects a report with root causes and corrective actions by 18 September, a toolbox talk plan for the shelter team and the WHS Incident and Hazard Register updated with actions and owners. The Chief Executive Officer will review the report at the monthly welfare and safety review.
Subject: Investigation of the Duke bite incident WHS-26-071
I am assigning you the investigation of the bite to Ben Takarangi on 8 September. Please read the incident report in the WHS Incident and Hazard Register, interview Ben and the kennel supervisor, review the Animal Register record for Duke and the Safe Animal Handling and Restraint Procedure, and identify what controls failed. I want a short investigation report with root causes and corrective actions, a toolbox talk plan for the shelter team and the register updated with actions and owners by 18 September. Marguerite will review it at the welfare and safety review.
Your brief
Investigate the incident systematically, identify the root causes and failed controls, recommend corrective actions with owners and dates, plan a toolbox talk and update the register so that the organisation can show it has learnt from the incident.
Work through it
You do not need to do it all. Doing a few of these will show you what the work is like. Your ticks and notes stay on this device.
What someone in this role would produce
- Investigation report with timeline and root causes
- Updated WHS Incident and Hazard Register entry
